This program that has been designed by a Clinical and Health Psychologist with years of experience working with adults who have experienced illness and are keen to learn how to manage themselves and their health to the best of their ability.

This program will involve learning about

  • understanding how illness can affect your physical and mental wellbeing,
  • how the health care system is interdisciplinary (i.e., multiple disciplines work together for example, doctors, physiotherapists, psychologists, dietitians) to help with navigating the system,
  • the importance of goal setting and getting active again (safely),
  • why brain-training and maintaining relationships is important, and
  • how to create your own interdisciplinary team and take your next steps.

The program is for adults who have been discharged from a medical inpatient ward within the last 6 months, recently diagnosis with a chronic illness (i.e., diabetes, fibromyalgia, COPD) or were recommended from their health providers to attend.
